NHS CARE DATA : modern data service for patient care. If you would like to read more about this project and your personal data, please go to http://www.england.nhs.uk/ourwork/tsd/data-info/ig/ or the NHS Choices website http://www.nhs.uk/NHSEngland/thenhs/records/healthrecords/Pages/care-data.aspx
In October 2013, NHS England started a nationwide publicity campaign similar to that for the Summary Care Records. This will involved sending information leaflets to every household in a four-week leafleting campaign informing patients about the "care.data" data extractions plans during January and February 2014.
After the campaign, patients were to have four weeks to tell their GP if they want to opt out of their personal medical data being forwarded on to the Health and Social Care Information Centre (HSCIC), before any extraction of patient data began. The extraction process has now been postponed until October 2014 because of concerns that patients did not have sufficient information to make an informed decision about their medical data.
NHS England said the leaflets would clearly set out how "care.data" will work and how patients can opt out, while also setting out ‘the benefits it will bring’. "Care.data" will see HSCIC routinely extracting identifiable patient data from GP practices and hospitals, which can then be shared in identifiable or de-identified forms to other sections of the NHS and to third parties, such as researchers.
The Local Medical Committee has provided practices with a summary of all the various data projects underway and copies are available in the practice or via this link CARE_DATA_EXTRACTIONS_ETC_-_NOTICE_24.2.14.doc
If you do not wish your medical data to be used in this way then please complete the form below and return it to the surgery. The same form can be used for family members if this is relevant. CARE_DATA_OPT-OUT_LETTER_15.1.14.doc You can opt out of the data collection at any time.
