New Patient Registration
Registration forms for new patients are available from reception. Once these have been completed and returned, your application will be processed in the surgery. The information is electronically relayed to the NHS authority for new registrations and your records requested from your previous GP. If you have a medical card with your previous GP details this is helpful in providing details needed to process your records. If you cannot find this then an NHS number will also be helpful.
A patient is registered with the practice, not an individual doctor. We appreciate that some prefer to see a specific doctor for continuity and the practice is happy to accommodate this with the preferred doctor; it will not always be possible however, particularly if your need is urgent or the doctor is away from the practice. .
In line with advice from the NHS ruling body we now ask for proof of identity and residence within our practice area at the point of registration - the receptionist will be able to advise you further if this is problematic.
Medical treatment is available from the date of registration - please just ring reception for further information.
